Form Requirements

Each group is required to submit certain forms in order to be eligible to compete at a CWEA-sanctioned event. These requirements are different for Scholastic Groups and Independent Groups. Questions concerning forms should be directed to [email protected].

Participating Group Master Agreement

The Participating Group Master Agreement is required of all groups competing in a CWEA-sanctioned event. This form must be on file with the circuit prior to the closing date for your first contest, or your group will not be scheduled to compete. This form has replaced the Liability Release Form and Scholastic Eligibility Form previously required of all scholastic groups.

There are two versions of the agreement – one for groups whose students all attend the same school, and one version for groups that are combining students from multiple schools. Please make sure that you complete the correct version of the form to avoid any issues.

Primary/Secondard Contact: All groups are required to disclose a primary and secondard contact. The primary contact should be the group's director or person who is responsible for the group on a day to day basis. The secondard contact should be the credentialed school official responsible for the group. These two contacts must not be the same person.
Announcer Script Information: Details entered here will be used to generate your Spiel Sheet for events. You may customize details such as group name, show title, however no deviation from the standard script used for all groups will be allowed.

All independent groups are required to submit proof of insurance coverage in order to perform at a CWEA-sanctioned event.

Scholastic Member Release Form

Section 1.1.4 of the CWEA Policy Manual requires that all student who are actively participating with a CWEA member high school as of August 1 to participate in an independent group to file a letter of release with CWEA provided that school has filed a letter of intent with CWEA by September 15. The letter of release must be signed by the student, a parent or legal guardian, the independent group director, scholastic group director, band director and the school Principal. If the scholastic group has not filed a letter of intent with the circuit by September 15, then no release letter is required.
